I do not bill standard health or medical insurance directly at this time.

Superbills

Superbills are available upon request for clients who wish to submit claims to their insurance provider for possible reimbursement.

To include diagnosis (ICD-10) codes on your superbill, I must have a current referral or prescription from your physician or other qualified healthcare provider. The referral is used to verify the diagnosis codes and must remain current. If it expires, ICD-10 codes will be removed from future superbills until an updated referral is received.

Please note that obtaining and maintaining a current referral is the client's responsibility. I am unable to contact your physician or healthcare provider to request updated referrals on your behalf.

Auto Injury (PIP) Billing

I offer in-house billing for approved Personal Injury Protection (PIP) claims. To schedule under PIP you will need to fill out a seperate intake form. Please fill out the standard massage form and mark Injury or referred and I will send you out the Medical Intake once approved I can contact you to book

Your healthcare provider may fax referrals or prescriptions to 509-888-4839.

HSA & FSA Payments

HSA and FSA cards are accepted for eligible massage therapy services. Please check with your HSA/FSA provider to verify your specific benefits and coverage.
